Government health plan eligibility

Peak Family Health is a direct-pay medical practice and is not enrolled with Medicare, Medicaid, TRICARE, or other federal or state government-sponsored health programs. Patients enrolled in those programs are not eligible for cash-pay medical services through Peak Family Health under the current practice policy.

Before you book primary care

Do I need a membership?

Telehealth and In-Office visits are available to members and non-members. Membership is available only to established Peak patients: book a Telehealth or In-Office visit and be seen by Britney first. Peak can then help you enroll privately. In-Home visits require Peak membership.

How do I choose a visit type?

Choose Telehealth for appropriate virtual concerns, In-Office for hands-on evaluation, or In-Home as a Peak member within the service area.

How do I book?

Do you accept insurance?

Peak is a direct-pay practice. Please review the government health plan eligibility notice before booking.

Membership starts at $200/month for one person, with each additional family member $75/month. See the pricing page for household options and separate visit rates.
